Underground Storage Tank Program

If you own or operate underground fuel storage tanks in California, you are responsible for ensuring that the following forms are filed with your CUPA by uploading them in CERS.

• Tank Closure Form C (PDF)
• Monitoring Plan Form D (PDF)
• Response Plan Form E (PDF)

Financial Responsibility forms are required to be updated and uploaded annually:

• Financial Responsibility (PDF)
• Financial Responsibility Chief Officer (PDF)

UST Forms
The forms below are required by the City of Healdsburg / Sebastopol CUPA if you plan to remove, install, or modify a UST system.

The following forms must be completed for underground storage tank removals or temporary closure of USTs:
